Improve caching in Docker FHIR Proxy

Currently, the Docker FHIR store caches the whole resource when a query needs to be paged unlike the InterSystems version which just caches the resource IDs.


The Docker version should be changed to just store the IDs and link back to the main Resource table if and when the resource is required. This will save on a lot of space I/O.

    thanks Steve, this looks like an efficiency gain, will promote to a feature so that it can be sized and considered alongside other priorities
