Develop the Interweave product to allow data sharing between different instances/regions within the consortium

In Nottinghamshire we have a large crossover with other bordering ICS bodies, for example LLR, Derbys and South Yorkshire.

Our immediate use case is to have the ability to share with colleagues in the Bassetlaw area (including acute trusts). We have defined three distinct use cases to support this request:

Discharge To Assess

Nottinghamshire County Council North D2A Team are part of the Transfer of Care Hub, and will be expected to put in place interim care in order to support a timely discharge. They will need to have visibility of the current inpatient encounter (e.g. fit status, expected safe for discharge date), along with any key findings, observations, medications changes that have resulted from the stay.

Community Health / Social Care

Both community health and social care teams received requests to support individuals (e.g. from GPs, concerned members of the public, etc). In order to ascertain whether involvement is required, and if so to what extent, it is useful to be able to see details of hospital visits and future appointments

Admission Notifications

Both community health and domiciliary care teams waste time visiting individuals who are not home as they have been admitted to hospital – either as an emergency attendance, or an elective visit that they forgot to inform them about. The ability to subscribe to Hospital Encounters and identify potential clashes with social care / community health appointments, could reduce wasted visits, allowing resources to be redeployed to productive work

    Hi Sean, this is a biggy but definitely something we intend to do, and the use cases help. I'll promote to the backlog so we can nail the design and consider the sizing and scheduling
