Implement patient merge functionality

Patient merges are not currently catered for in the FHIR proxies or PIX. If a patient merge occurs at a provider, the new identity must replace the old identity in all references. Work is ongoing for the FHIR R5 specification to define the $merge operation (https://confluence.hl7.org/display/PA/Patient+Merge+and+Link)

    this is an important standardisation to ensure there are not gaps in patient data available to consumers. Patient merges are a relatively frequent occurrence across all ED and acute admission departments so all/any newly onboarded providers would benefit from this development.
