FHIR Appliance and Aggregator to support _summary=count

This search parameter would allow systems like onboarding to display the number of resources persisted in a FHIR store/appliance.

Also useful to provide the same information to data providers who use the regional FHIR appliance (ensemble or node).

The aggregator can dispatch this query to participants and, for those that support it, aggregate the results to give a regional summary.
